Please fill out the following form with details and amounts for reimbursements

within two weeks of the event.

NB: Upon submission of the form, you will be able to upload your W-9 and receipts.

Notes:

Amounts are subject to per-event maximum.
A “local lecture” is defined as a lecture given within 45 minutes travel time of the speaker’s residence.
If you have already sent in your W-9, you do not need to send it again, except in the event that your information (such as your address) has changed.
